In an emergency, the last thing you should worry about is paying for the ambulance bill.
Your support helps fund our nonprofit mission of improving the quality of life for the people who live in, learn in, and visit Prince Edward County. Since 1969, we have provided primary emergency medical service, life-preserving care, community education, and continuous quality improvement.
We have TWO different membership types:
Family Membership
A family squad care membership costs $50 and covers you and members of your immediate family (spouse, minor children, and parents) who live in the same household as listed on your application.
Business Membership
A Squad Care Business membership offers coverage to all employees and volunteers while at any location where an employee is performing job-related duties. Just like a regular Squad Care membership, a Squad Care Business membership covers the out-of-pocket expenses for the uninsured portion of PEVRS charges including any costs for co-insurance and deductibles not covered by insurance for all employees. Emergencies happen, and they can be a financial burden. Squad Care Business means that anytime an employee is “on the clock” and requires emergency transportation by PEVRS, they will not be responsible for any portion of the ambulance bill not covered by insurance. A Squad Care Business Membership does not cover emergencies that may occur outside of working hours.
Business Memberships Costs:
- Bronze (1-10 employees) = $100.00
- Silver (11-25 employees) = $200.00
- Gold (26-49 employees) = $400.00
- Platinum (50+ employees) = $500.00
Membership Fees
The annual membership fee for my PEVRS Squad Care covers out-of-pocket expenses for the uninsured portion of charges for medically necessary ambulance transportation to the hospital, including any costs for co-insurance and deductible not covered by insurance. Transportation from the hospital to another location is not covered by this membership, nor is ground or air transportation provided by another agency. Additional funds beyond $50 will be considered a donation to your local rescue agency.
Membership Terms
I understand the Squad Care plan covers members of my immediate family (spouses, children, or parents) who live in the same household as listed on my application or a single-individual household and that the plan covers only medically necessary ambulance transportation by the agency that ends at Centra Southside Community Hospital.
I also understand that Squad Care is not insurance and that PEVRS will claim payment from my insurer or third-party agency (i.e. Medicare). I understand that I may be asked to help PEVRS collect these benefits or payments, and I agree to help fill out any necessary forms, provide necessary signatures, and perform other tasks that may be reasonably required for PEVRS to collect payment. If I receive a payment directly from an insurance company, I agree to immediately forward that payment to PEVRS under this agreement.
By signing the reverse, I request that payment of authorized benefits be made on my behalf to Prince Edward Volunteer Rescue Squad, Inc. for any ambulance services provided to me by the agency. I authorize any holder of medical information or documentation about me to provide accurate information to the agency if needed to determine the benefits payable for related services provided by the agency now or in the future.
Effective Dates
I understand that my membership is effective upon receipt of full payment and signed membership contract, beginning January 1, 2025 through December 31, 2025, unless otherwise specified. I also understand that this membership fee is non-refundable, and the membership is non-transferable.
